How do you know the person or group you are nominating?

We need to be able to invite your Nominee (plus Guest) to the Awards Event. Please seek relevant permission to give us as many contact details as possible of your Nominee's Parents/Carers (or your Nominee's contact details if 18+). Email addresses are most helpful.

If you are nominating a group, please give all individuals' names so we can issue their invitations and certificates. It is fine to give First names and initial of surnames if this is more appropriate. We cannot issue individual certificates without knowing names of those involved.
